I use Rogue Amoeba's Audio Hijack Pro to do all you want on Mac, and they suggest this for windows:

Hi Tom-I have used a free program from a company called Applian
Technologies for the last year or so. It is called Freecorder, and works
very well. The company also sells an application called Replay A/ V, which
sounds like it does what you want. Look here:
