Karen,

We have a long-standing brittle books program at the University of Maryland 
Libraries Preservation Department.  Our procedures are on the Web at 
http://www.lib.umd.edu/TSD/PRES/brittleunit.html
The process is managed with an Access database and our form is an Access 
report.  I will fax you copies of it.  We are fortunite to have a professional 
librarian, Susan Koutsky, to head that unit.  Sue's email address is

the Access database if you like.
