[PADG:1468] selection revision

Dear PADG:  I would like to ask a favor of PADG subscribers.  I am in the 
process of revising our selection form/process for brittle books and others 
than cannot be reasonably repaired or rebound.  To assist this I would very 
much like to attain copies of selection forms and/or procedures from as 
many academic libraries as possible.  If you are willing to pull this on my 
behalf and forward it to me, either electronically or through the postal 
service, I would very much appreciate it.
