Publication sought

I am looking for "Business Handbook for Conservation Professionals".
I have a photocopy of the cover page and table of contents that states:

   "Published by Conservators in Private Practice, a Sub Group of
    the American Institute for Conservation, copyright June 1996.
    Additional copies may be purchased from the office of the
    American Institute for Conservation of HIstoric and Artistic
    Works"

CoOL searches produced no results. AIC publications has no record of
its existence and CIPP is still searching their records.

Can anyone tell me how I might obtain a copy? It looks like a
terrific publication.
