Here at the National Museum in Krakow, Poland, we use just our own
template for preservation documents--in Word. Currently there is a
project for unifying this documents within all National Museums in
Poland--for better understanding and clearness while lending
objects.

There is a guideline for conservation documentation put up by the
National General Conservator's office to be used by all conservation
specialists. It is an obligatory document accompanying all major
restorations and conservations. It includes:

    description of the object
    history
    technology description
    storing description
    state of preservation
    conservation programme
    description of conservation course
    advice for user
    drawings, photos etc.
