I would suspect one of two ways:
1) Keep track of the tunes (via a set list) and submit appropriate payment
to HFA/CMRRA/usw. via whomever keeps track of the band's "business end"...
2) Cross their fingers that they aren't caught by the agency above, and
keep ALL the money!
Obviously, the total sold will dictate the choice...if Joe Gabroni and
his Blues Knaves sell a handful of copies at their local shows, it's
unlikely to be noticed, while if <name of major act> sells large
quantities at their arena shows, they'd probably better pay up!
I just wish this sort of "on the spot" mass-production had been
practical for cassettes. I taped most of my shows from 1986 to
1988, and have no doubt our fans would have gladly bought recordings
of what they had just enjoyed...
