[PADG:218] Heritage Health Index Deadline Extended Until October 26, 2004

(Please pardon cross postings.)

In August 2004, a sample of 15,000 archives, historical societies,
libraries, museums, and scientific research organizations received the
Heritage Health Index survey. Heritage Preservation, in partnership
with the Institute of Museum and Library Services (IMLS), is conducting
the Heritage Health Index. Institutions of all types and sizes in all
states and territories were selected to participate in this survey that
will-for the first time-gather comprehensive data on the condition and
preservation needs of U.S. collections. The deadline has been extended
until October 26, 2004, so that as many institutions as possible can
respond to this historic survey.

Respondents may reply using the Heritage Health Index survey booklet
they received by mail or at www.heritagehealthindex.org using the
password provided in the survey package. If your institution needs
another copy of the survey booklet or password, contact Kim Streitburger
at RMC Research Corporation at kstreitburger@xxxxxxxxxx or 800-258-0802.

If you have questions about the Heritage Health Index, contact Kristen
Laise at klaise@xxxxxxxxxxxxxxxxxxxxxxxx or 202-233-0800.
