Conservation workflows for medium rare library materials

I would be interested in hearing from colleagues in library
conservation labs who have a workflow or documentation protocol for
"medium rare" collection materials, i.e. materials that are more
special than your average general collections/circulating book, but
not necessarily warranting full-fledged special collections
conservation treatment.

I am particularly interested in

    What sorts of materials typically fall into this workflow and
    how you delineate this category of treatment

    What level, if any, of photo-documentation you conduct

    If you involve curatorial approval of treatments as part of your
    process

    If you use narrative, "check box", or other types of treatment
    documentation and if you could share any documentation forms you
    use, that would be most appreciated
