Insertions in books

Regarding best practices for the placement of archival book cards or
flags in books: Should the book card or flag be placed at the title
page, somewhere in the middle of the text block or at the back
cover?

If the book card or flag has a barcode on it, is it a good idea to
have the barcode at the top of the book card or flag to avoid having
it touching the paper?
