The following survey is part of a research project investigating the
current trends in selection of library materials for preservation
and/or conservation treatment including repair, reformatting (for
preservation purposes) and enclosures in light of the current
availability of digital surrogates and other institutional holdings.
Participants from research libraries working within preservation
programs are encouraged to participate in an anonymous, online
survey at:

If possible, we encourage only one response from each preservation
program to avoid unnecessary duplication of data.  The information
will be used both to help inform local practices at the University
of Illinois, as well as contribute data to a professional
publication that will have the potential to inform the larger
preservation community.

Your decision to participate or decline participation in this study
is completely voluntary and you have the right to terminate your
participation at any time without penalty. You may skip any
questions you do not wish to answer. If you want do not wish to
complete this survey just close your browser.

Your participation in this research will be completely confidential
and data will be averaged and reported in aggregate. Possible
outlets of dissemination may be peer reviewed journal articles and
professional conference presentations.  Although your participation
in this research may not benefit you personally, it will help us
understand how the effects of available digital surrogacy and shared
print repositories are changing the way preservation decisions are
being made.  There are no risks to individuals participating in this
survey beyond those that exist in daily life.
