How not to preserve photographs

For a forthcoming workshop entitled, "How Not to Preserve
Photographs," I am looking for examples of instructive errors made
in the past in the field of photo conservation, whether through
accident, ignorance, or other causes.  Any help along these lines
would be appreciated,
