Rane has a downloadable article discussing the nuts and bolts of interfacing 
to the Plain Old Telephone Service (POTS). The article also contains a 
schematic for a simple interface, and various transformer sources.  I have 
built the unit using a decent quality Hammond transformer, and it works very 
well.
